School Safety – Sign-In Sheets

Who is visiting your site?  Do you know?  Does anyone know?

Vital for the safety of children as well as staff at every site, are the sign-in sheets.  Every site should have a binder for visitors including delivery personnel, AMC reps from other sites, ACS supervisors, evaluators, or grandmothers and cousins coming for the grandchildren’s birthdays.  Therapists who provide services for children at our site must complete provider information forms (in forms and links) and sign in to the building, as well as into each individual classroom, whenever they come to services a child.  Directors have copies of the forms and they are on the forms/links page of this site.
